Self-Care for the Caregiver

Jane Meier Hamilton MSN, RN, a nurse for 35 years and family caregiver for 20 years, has practiced clinically, taught psychiatric nursing, published numerous professional journal articles and co-authored Survival Skills for the New Nurse (JB Lippincott, 1986). She established Hamilton Consulting in 1979 to address the needs of health care professionals, later expanding her practice to include caring for family caregivers. She has consulted with more than 150 organizations and presented over 2,500 workshops and keynote addresses nationwide. In 2008, she founded Partners on the Path to help professional and family caregivers preserve their health, well-being and capacity to care. Participants will receive a free copy of her book Journey of a Lifetime: The Caregiver’s Guide to Self-Care (Infinity 2010), an original, research-based, stress-coping model for self-care.

For our May 8 meeting, we have two events in one!

1. Presentation on Financial Planning for your special child's future, by Branden Sacks, a financial planner with expertise in helping families like ours. This is a great follow-up to the wonderful April 10 presentation by Hillary Freeman on the legal aspects of future planning.

2. Display of books on special needs, organized by Margaret Brunton of the Chestnut Hill Library. She is gathering books from throughout the Philadelphia library system and will briefly discuss how to find books and other resources offered through your local library.

Note the change from our regular time: 10 am to 12 noon (refreshments served at the end, so please arrive promptly at 10!).

The SPEC meeting is early so you can come over to Finley Recreation Center at 1pm for "An Afternoon Delight" ~~ a dance party fundraiser for Autism Sharing and Parenting. Of course, the event is open to all. Join us, have some fun! Please, if you are so inclined, pass this on to anyone who you think may enjoy an afternoon of dancing. The $25 donation includes childcare, if a family needs to bring the kids.

PA House Bill 1665 provides special funding guidance for PA approved private schools (including the four chartered school for the deaf and blind, which includes my daughter's school :-) -- please contact your representatives and let them know how important it is to support this legislation (as well as your family and friends); you can determine your state senator’s and representative’s contact information by clicking on the following link and putting in your home address: www.congress.org -- some points to consider:

H.B. 1665 is important to schools that educate children with disabilities because it will help our school to keep up with the funding for special education and basic education. Both basic and special education are likely to have significant increases for 2009-10 because of federal stimulus dollars, which can’t be used for the Approved Private School for the Children with Disabilities. Because the increase for special education is from federal dollars, the line item for special education this year is flat lined. This means that in 2010-11 there will be no increase for our school, unless H.B. 1665 is enacted.
Our Approved Private Schools teach children with the most severe and complex disabilities – students who come with a variety of disabilities and in many cases, multiple disabilities. A student population with one or more complex and severe disabilities means higher costs to educate.
The bill applies only to those fiscal years in which special education is flat lined. In such a year, the increase for our school would be the average percentage of the increases for special and basic education in the last year in which there was an increase for special and basic education.
H.B. 1665 will require no funding for 2009-10 as our appropriation for the 2009-10 is based on last year’s special education line item.
(When communicating with Democratic House members) – “ Please tell Appropriations Chair Dwight Evans and your floor leadership, Majority Leader Eachus and Speaker McCall that you would like H.B. 1665 amended into the omnibus school code bill that will pass with the budget.”
(When communicating with Republican House members) - “Please tell Appropriations leader Mario Civera and your floor leader, Sam Smith, that you want H.B. 1665 amended into the omnibus school code bill that passes with the budget.”

Description, Eligibility Criteria & Instructions for Respite Services

The Department of Behavioral Health is providing three unique styles of Respite Services in an effort to offer families caring for a child with behavioral health needs an opportunity for a much needed break. The three Respite Services being offered by the DBH are the Boy’s and Girls Club Saturday Afternoon Respite, Wordsworth Weekend Residential Respite & Silver Springs Weekend Overnight Residential Respite. All of the DBH Respite Programs are FREE. The DBH Respite Services are not an entitlement and availability is limited and on a first come first serve bases until June 28^th 2009 or until funding is exhausted.

The DBH Respite Services are a recreational support and NOT treatment. The DBH Respite Services are not designed to be an alternative for crisis intervention, emergency evaluation, inpatient care or any other form of Behavioral Health treatment. The DBH Respite Services are NOT designed for children, who are verbally/physically aggressive, sexually inappropriate, chronic runaways, medication non-compliant, suicidal/homicidal or non-compliant towards authority figures on the day of respite. If your child present with any of the above he/she WOULD NOT be appropriate for the DBH Respite Services at this time. However, respite services can be considered once your child’s behavior improves. In addition, the DBH Respite Services CANNOT accommodate children who are sick i.e., fever, vomiting, diarrhea, contagious - pink eye, impetigo, lice or ringworm etc. If children are brought to respite sick or contagious their parents will be called to immediately pick them up.

Eligibility Criteria:

You MUST register your child with the Department of Behavioral health to determine eligibility.
You MUST submit a separate registration form for each child that you wish to register.
Your child MUST have a Mental Health Diagnosis provided by a Behavioral Health provider.
Your child MUST currently receive Behavioral Health services.
Your child MUST currently reside in Philadelphia County.
Your child CANNOT reside in any placement at the time respite is being requested. i.e., residential placement, juvenile placement, foster care or group home
Only children who are eligible and registered can utilize any of the respite services i.e., siblings, friends who are not eligible cannot attend respite.
The agency providing respite services MUST be notified 24 hours prior if the services are no longer required. Failure to do so may result in you no longer being considered for future respite services
The registration form MUST be signed by appropriate persons your child WILL NOT be registered if the registration form is not completed and signed.
If your child receives TSS services the TSS may be able to accompany your child to the respite program.
We recommend that you register with the Department of Behavioral Health as soon as possible to make sure your child is approved for respite services and that you get on the list.
We recommend that you not wait until just before needing respite services to register.
We recommend that valuables not be brought to respite. No one is responsible for any items loss or damage.

Boys and Girls Club Saturday Afternoon Respite Program

The Boys and Girls Club Saturday Afternoon Respite Program will begin in January 31, 2009. This program will rotate each Saturday at one of five different Boy’s and Girls Clubs located throughout the City of Philadelphia. The hours of operation are 12 noon- 5:00 p.m. The Boys and Girls Club is a safe child and youth friendly facility, with six trained staff available at all times. The Boys and Girls Club Saturday Afternoon Respite Program can accommodate up to 35 children and youth each Saturday ranging in ages 6 to 15 years old. The children and youth will engage in age appropriate activities and will be grouped accordingly 6-8yrs, 9-11yrs, and 12–15yrs. All children and youth that participate in the Boys & Girls Club Saturday Respite Program will ultimately receive a membership to the Boys & Girls Club which will allow them to participate and utilize the activities in any of the twelve Boys & Girls Clubs in Philadelphia until June 30, 2009. All parents and children who register to participate in the Boys & Girls Club Saturday Respite Program are required to attend a one time only mandatory 1 hour orientation before the child or youth can be admitted to the respite program. Lunch and snacks will be provided however children with special dietary requirements may need to bring their own food and beverages The Boys & Girls Club Saturday Afternoon Respite Program is FREE until 5:00 pm ONLY!

Saturday Respite Activities: Arts and Crafts, Sports, Outdoor play (weather permitting), Computers, Video games

Instructions:

Parent/Legal Guardian must pre-register in person with the Boys and Girls Club Monday through Thursday from 7:00 a.m. until 7:00 p.m. prior to the Saturday they would like their child to attend.

Registration includes: reviewing policies and procedures, signing a membership form.

Parent/Legal Guardian MUST submit an updated Child Health Assessment (for membership purposes only not required for the Respite Program).
Parent/Legal Guardian MUST notify the Boys and Girls Club in advance if their child will be unable to attend. Failure to do so may result in you no longer being considered for future respite services.
The Boys and Girls staff WILL NOT administer medications however they will hold all medication and supervise any child able to self medicate.
Parent/Legal Guardian MUST accompany their child into the building and sign them in.
Parent/Legal Guardian should bring photo ID when delivering and picking up their child.
Parent/Legal Guardian MUST come into the building to sign their child out and show photo ID before they can leave the program.
Anyone other then a parent delivering or picking up a child MUST present photo ID. And they must have written permission submitted by the parent in advance.

Parent/Legal Guardian are FULLY RESPONSIBLE for paying all LATE FEES if their child is not picked up by 5:00 p.m.

Wordsworth Weekend Respite Residential Program

Wordsworth Weekend Respite Residential Program will begin February 6, 2009. The Wordsworth Respite Program will operate from February 6, 2009 through June 28, 2009 or until funds are exhausted. Wordsworth will provide both treatment foster care and host homes for the respite program. Wordsworth will ensure that all respite providers are trained foster parents and have their required clearance. In order to participate in the respite program children must first be registered with the Department of Behavioral Health and determined to be eligible. Once registered and approved parents can contact Wordsworth directly to arrange for respite services. This is not an emergency program. Wordsworth respite services are available Friday 7:00 p.m. until Sunday 7:00 p.m. only. A respite stay can be a full day, overnight, or a weekend. Wordsworth Weekend Respite Residential Services can be accessed a maximum of 2 calendar days anytime from February 6, 2009 – June 28, 2009 (a 24 hours respite stay that continues into the next day for a total of 4 hours constitutes 2 calendar days) i.e., Saturday 8:00 a.m. until Sunday 12 noon. Wordsworth will be accepting children on a first come first serve basis and will do their best to match the appropriate respite home to the child’s specific needs and personality. There is however no guarantee that every child will be matched for every weekend request. Most of the respite homes are in Philadelphia County, however there are several located in nearby counties. Parents are responsible for delivering and picking up their children. Both the parent and child are subject to follow the intake requirements and the policies and procedures of Wordsworth. Meals will be provided however children with special dietary requirements may need to bring their own food and beverages. The parent/legal guardian must pick up their child on time. Any late fees must be paid in full by the parent at the time of pickup.

Instructions:

Parent/Legal Guardian MUST contact Wordsworth 2 Monday’s prior to the date of the needed respite service by 5:00 p.m. to request Wordsworth Respite Services.
Parent/Legal Guardian CANNOT schedule respite care for their child more than 30 days in advance.

Silver Springs Weekend Only Respite Residential Program

Silver Springs Weekend Only Respite Program will operate from January 2009 through June 28, 2009 or until funds are exhausted. Silver Springs will provide treatment foster care homes for the respite program. Silver Springs will ensure that all respite providers are trained foster parents and have their required clearance. In order to participate in the respite program children must first be registered with the Department of Behavioral Health and determined to be eligible. Once registered and approved parents can contact Silver Springs directly to arrange for respite services. Parents can access respite care through Silver Springs during weekend hours only, for a minimum of 2 calendar days or 30 hours (24 hours of one day and at least 6 hours of a second day will be viewed as two calendar days) and a maximum of 48 hours from Friday 7pm through Sunday. Silver Springs Weekend Only Respite Program is not an emergency program. Silver Springs will be accepting children on a first come first serve basis and will do their best to match the appropriate respite home to the child’s specific needs and personality. The Academy of Motion Picture Arts and Sciences has announced that it will give Jerry Lewis its Jean Hersholt Humanitarian Award on February 22, 2009. Disability rights activists object to this award. During his decades of hosting the Labor Day Telethon, Jerry Lewis has helped to perpetuate negative, stereotypical attitudes toward people with muscular dystrophy and other disabilities. Jerry Lewis and the Telethon actively promote pity as a fundraising strategy. Disabled people want RESPECT and RIGHTS, not pity and charity.

In 1990, Lewis wrote that if he had muscular dystrophy and had to use a wheelchair, he would "just have to learn to try to be good at being a half a person." During the 1992 Telethon, he said that people with MD, whom he always insists on calling "my kids," "cannot go into the workplace. There's nothing they can do." Comments like these have led disability activists and our allies to protest against Jerry Lewis, and against the Telethon. We've argued that the Telethon promotes pity, a counterproductive emotion which undermines our social equality. Here's how Lewis responded to the Telethon protesters during a 2001 television interview: "Pity? You don't want to be pitied because you're a cripple in a wheelchair? Stay in your house!"

Disability rights activists have launched an online petition to protest the Academy's decision and to demand the award be canceled. To sign the petition, go to

Please share the archived handouts (.zip of pdf files, 420 K) with anyone who may be interested (i.e., potential consumers and/or providers). These handouts are also available online at www.autisminpa.org.

Please note that the “Provider Packets” mentioned in the “Provider FAQ” document will become available on June 16, 2008. Hello from my son Quinn's music class (they provide free wireless while I watch from the "parent room" outside the studio) -- and Quinn really enjoys the class, interaction with specific guides.

My wife forwarded a note about advocacy for HB1150, providing insurance for children with autism -- I recognize my friend from HS Tom Murt as a sponsoring PA Representative, and I am working to contact my Representative Greg Vitali (or ask that Tom give him a message, as if I had that kind of access :-).

On April 1, 2008 at 10 AM, Capitol Building Complex (Harrisburg), Hearing Room 1 of the North Office Building, the Pennsylvania Senate Banking & Insurance Committee will conduct a public hearing regarding the merits of HB 1150 -- there seems to be a balanced panel of advocates, including David Gates of the Pennsylvania Health Law Project -- I was initially surprised see Rachel Brooks, Miss Pennsylvania 2007, listed as an advocate due to her brother with autism -- from my quick read it seems not like another "throw money at it" approach, but a directed investment in helping kids with autism in an early (and thus efficient) way -- more and up-to-the-minute information is available here.

Renee Henderson, is parent to a nine-year-old girl on the spectrum with severe language delays. Renee has supplemented her daughter's public education and therapy with "home schooling" since her diagnosis. She has nurtured her natural gift with seminars, workshops and conferences covering all areas of behavioral therapy and education for children on the spectrum

Gail Stein, L.C. S.W., is Director of the Variety Club for Children Family Autism Resource Center. She has been key in orchestrating countless autism community events including the Variety Sesame Place Day for children on the spectrum and Variety Club Fun Days and Summer Camp

What is ASP?

ASP is a volunteer run, Community-based, support group with the following goals

Working the Web ...

... this week, yours truly will be bring a laptop and hope to start (perhaps incite?) a discussion about how a computer and an Internet connection can be used to make life with our kids "better", a term we all define differently. Besides this Blog, I hope to show people the benefits of search, social networks and communities, and other IT tools than can help; but as I tell my students, the computer is dumb, people need to be able to ask the right questions, process/filter the information, and know the limits.

The presentation will be brief, and is dependent on the technology available at the CH Library, as well as the interests and patience of those in attendance -- no need to bring a computer, but you can, and certainly bring your questions as I learn most from trying to find the answers!

Ruth Landsman this SAT ...

This month we welcome Ruth Landsman, director and founding member of the Parents Exchange, an information, referral and advocacy organization. Ruth has been tirelessly advocating for the rights of children to special education for nearly two decades.

Ruth has immeasurable experience in the legal and practical issues parents face in seeking appropriate services for their children. As a mother of a young adult who has special needs, Ruth also brings her perspective as a parent.

The discussion will focus on Transitioning, particularly from preschool to kindergarten, and from school years to ESY -- bring your questions!

Air Travel: an impending adventure

My family is taking me to Portland, Oregon USA in the spring, so I have been doing some work on airfare, availability, hotels, light rail -- and accessibility. Fortunately, we're going to Portland, where I have visited a few times (great restaurants), and it seems to be an accessible city. The airport (PDX) is smaller than many, but open and easy to navigate (and free wireless). The MAX light rail provides cheap and accessible transportation from PDX to center city; once there it is free to use in the downtown area, and that includes much.

I am more concerned about getting to Portland from PHL, the home of the "losingest" team in professional sports (though I was pleased to find the Philadephia Airport has an ADA/Text-Only View, and Accessibility/ADA page and even an accessibility guide; you may also call 215-937-5499, or TDD# 215-937-6755, to request the "Getting Around Guide." -- cool).

I started looking at airline websites, and found a few helpful things, like

United's quick access for customers with special needs
Southwest's Customer Service Commitment (PDF)
US Airways page for Special Needs and for "Disabilities" (as if you can really separate this from the person; I am sure they mean well, and mean "people with disabilities" :-)
Delta's Special Travel Needs page
and perhaps most importantly, the Air Carrier Access Act (another PDF); my wife also forwarded the Aviation Consumer Protection Division of the DOT site

I invite -- no, encourage -- any reader who has traveled with a disability or a person with a disability to share their experience, and any effective tricks, phone numbers or other resources that we all can use. I will certainly share mine.

More Hurdles for our Families

I doubt I can keep up the pace of a post a day, but I just could not resist the front page story in the NY Times about "Disability Cases Last Longer as Backlog Rises," with reports of waits as long a three years (although 2/3 of those initially rejected win their cases, so persistence pays off more than not -- but you knew that already :-).
I hate to be cynical (but willing to do so for this cause), there's more incentive to reject as much as possible -- the articles cites some of the "incentives," like people do not persist past the first rejection, do not get help, lose resources like homes due to bankruptcy and other obstacles while waiting, or simply die in the queue -- plus, from what I can read, there are no penalties (i.e., "dis-incentives") for the SSA to face should they reject improperly. Surprise, the backlog, the frustration (but a more fiscally sound SSA that politicians can brag about).

Seems the only disincentive I can see is the political fallout from frustrated citizens without disability support who can still vote, and will certainly vote this issue as it impacts their lives directly. Not only would I vote this issue, but so would all of my daughter's family, friends, friends of friends, and citizens whom she has met at awareness and fund raiser functions.

Also in the NY Times, an article that concluded with the following paragraph:

“I think there are a lot of parents of kids with these diagnoses who have at least a little bit of the traits their kids have,” Mr. Schwarz said. “But because of the stigma this society places on anything associated with disability, they’re inhibited from embracing that part of themselves and fully leveraging it to help their kids.”

Like the disability itself (or the administrative bureaucracy) isn't enough of a fight?

Meeting Summary with OT Heather

Today we had a nice crowd at the Chestnut Hill Library to hear from our invited speaker, Heather Bowditch, an Occupational Therapist with some extended preparation in clinical psychology (i.e., one of the good guys for parents :-) -- after our typical round of brief but important introductions, Heather began by reviewing some of the existing literature on how parents (and families) cope with the issues surrounding disabilities in the family.

Heather noted that some believe the way people cope with disabilities is related to similar stages used to cope with death (Kübler-Ross model): namely, denial, anger, bargaining, depression, and eventually acceptance. There was much discussion about this topic among those in attendance, with a consensus that while there is some similarity, there is also difference (e.g., non-linear, many stages are repeated, some experienced concurrently).

Many also reported that since there is really no time to grieve, many simply grieve in a "functional" way -- in other words, either the feelings are experienced while we are caring for our children, or they are addressed quickly (if at all).

Certain stages, like anger, can provide "apparent power" to cope with a sense of powerlessness; we debated on the effectiveness or the appropriateness. Then there is acceptance, a final destination for those grieving about death, but somewhat different when there is no known end in sight. One person noted that acceptance needs clarification and context; we accept the disability, but not for such things as level of service or ignorance.

Personally, I was glad that Heather provided much space for discussion throughout her presentation. I was also very happy that there were great sandwiches available before the meeting so I could concentrate on the discussion :-). Heather can be contacted by email.

Welcome to SPEC Online

It has taken longer than I expected, but I am finally able to say, "welcome to SPEC online" -- for the last seven years, this group of motivated parents (read later to see why we are motivated) has met monthly during the academic year to exchange ideas, explore advocacy options and obtain expert guidance for a set of dedicated members of the community to help families with children who have disabilities. For now, this blog will serve as the primary location for continuing dialogue about the topics that we start in the meetings but never seem to have enough time to finish.

Special Parents for Exceptional Children (SPEC) meets monthly when possible at the Chestnut Hill Library (photo right). Thanks to them for making space available on Saturdays, and click here for map and directions. We typically open with a light lunch provided by members and funded by a small grant from the Philadelphia Department of Health (I think -- corrections can be sent to me or posted as replies).

Each meeting has either an agenda (business to get done) or an invited speaker on a topic for the entire group. Past speakers have been Joe Scullin of UCP, Ruth Landsman of Ed-Law Center, and Doug Dye, attorney with expertise in special needs trusts and other financial and legal issues for our families. This group is as grass-roots as it gets, members suggest and invite speakers, make up the audience, and (eventually) work online.

Why we do what we do: yes, it starts out as for the children, but it as much for the parents; to learn, to support, to commiserate, and eventually to grow to accommodate the challenges we face -- more together than before the meeting.

Kudos to Jill Spector for starting this group, and to Jean McCoubrey for keeping it going for all these years -- I was supposed to put together a website, the easiest part of this project, so of course it took the longest time :-(. I will start as the first poster, and invite people to either respond to a given post or email me with content and other ideas. It may take time, but I am hopeful that this blog can help continue the work of SPEC outside of the meetings, perhaps even connect to other resources and groups.
